Does Medi-Cal cover TMS?
Yes — California’s Medi-Cal program covers TMS for treatment-resistant depression through many of its managed care plans. Coverage expanded as part of California’s behavioral health transformation, recognizing TMS as a real, evidence-based option for people who haven’t gotten better with medication alone.
How Medi-Cal TMS coverage works
Medi-Cal delivers mental health benefits through two pathways:
Managed Care Plans (most members):
- Plans like LA Care, Health Net, Molina, CalOptima, and others
- Each plan has its own TMS authorization process
- Contact your specific plan’s behavioral health department
County Mental Health (specialty mental health services):
- For severe, persistent mental illness
- Accessed through county mental health departments
- TMS may be available at county-contracted providers
Eligibility requirements
Most Medi-Cal managed care plans require:
- MDD diagnosis by a psychiatrist
- 2+ medication failures at adequate doses for adequate duration
- Prior authorization from your managed care plan
- In-network provider — must use a TMS clinic that accepts your specific Medi-Cal plan
Cost to members
Here’s the good part:
- No cost sharing for most Medi-Cal members. No copay. No deductible
- Some Medi-Cal plans with share-of-cost may require a monthly contribution
- Once authorized, TMS itself has no out-of-pocket cost
Finding Medi-Cal TMS providers
This is the biggest challenge — and it’s a real one. Not all TMS clinics accept Medi-Cal:
- Call your managed care plan and ask for TMS providers in your area
- Contact TMS clinics directly and ask if they take your specific Medi-Cal plan
- University medical centers (UCLA, UCSF, UC Davis, Stanford) may accept Medi-Cal for TMS
- Community health centers with TMS programs may participate
It takes some phone calls. But the coverage is there if you can find a provider.
Authorization tips
- Start early. Medi-Cal authorizations can take 2-4 weeks
- Have your psychiatrist document all medication trials in detail
- Denied? Request a peer-to-peer review immediately
- Medi-Cal members have the right to a State Fair Hearing if internal appeals fail
- California’s Department of Managed Health Care (DMHC) handles complaints about plan denials — they’re responsive and have enforcement power

How to Get TMS Approved
Call the number on the back of your insurance card and ask specifically about TMS therapy coverage. Get a reference number.
Gather records of your MDD diagnosis, all medication trials (names, doses, durations, outcomes), current PHQ-9 score, and therapy history.
Find an in-network TMS provider using our clinic directory. In-network clinics handle prior auth and know your insurer's requirements.
Your TMS clinic submits the prior auth request. Typical approval takes 5-15 business days. If denied, appeal — overturn rates are 60-70%.
What If You’re Denied?
Don't give up after a denial
TMS denial overturn rates are 60-70% on appeal. Steps to take:
- Request a peer-to-peer review — your psychiatrist talks directly to the insurer's medical director
- Submit additional documentation addressing the specific denial reason
- File a formal appeal with your state insurance department if internal appeals fail
- External review — most states allow independent external review of coverage denials
